Shared care for patients with cirrhosis: pilot trials in primary care

Cirrhosis is a chronic liver disease usually caused by alcohol or ‘the metabolic syndrome’ characterized by overweight, high cholesterol, and diabetes. Patients with cirrhosis can expect to live only 3–4 years from the day they are diagnosed. This poor prognosis is partly because cirrhosis is diagnosed late in its disease course, and partly because patients have or develop other chronic diseases. This project aims to improve the prognosis for patients with cirrhosis through a stronger collaboration between general practitioners (GPs) and hospitals. The project includes two randomized pilot trials that recruit participants through their digital mailbox (“e-boks”): The first trial explores the effects of inviting patients with diabetes to be screened for cirrhosis at their GP. The second trial explores the effects of offering cirrhosis patients an annual ‘care bundle’ at their GP. Moreover, we will establish two Patient Advisory Councils to further develop and implement these trials.
